Key Responsibilities

• Conduct thorough clinical assessments to evaluate the mental health status of patients in hospice care, involving both patients and their families in the process.
• Develop and implement individualized treatment plans that reflect best practices and adhere to evidence-based guidelines.
• Utilize a range of therapeutic approaches, including solution-focused therapy and grief counseling, to provide meaningful interventions tailored to each patient's needs.
• Ensure accurate and timely documentation of clinical records, adhering to privacy and regulatory standards.
• Engage in collaborative discussions with a multidisciplinary team, including physicians, nurses, and chaplains, to coordinate cohesive care for patients.
• Offer psychoeducation and support to families, helping them understand the psychological aspects of hospice care and ways to cope with their loved ones' transitions.
• Stay informed about advancements in mental health practice relevant to end-of-life care through continuous professional development and training.

Patient Population / Client Focus

Your primary focus will be on patients in hospice care, which involves addressing complex emotional responses to terminal diagnoses. This includes working with diverse populations that may encompass varying cultural backgrounds, life experiences, and family dynamics, necessitating a strong emphasis on individualized and culturally sensitive care.

Qualifications

• A Master's degree in Social Work from a regionally accredited institution is required.
• A valid LCSW license in the state of New Jersey is mandatory.
• Expertise in solution-based therapeutic interventions with an understanding of grief and loss is essential.
• Strong communication skills that facilitate effective interactions with patients, families, and interdisciplinary team members.

Preferred Experience

• Experience in a hospice, palliative care, or similar setting is highly valued.

Compensation and Benefits
