Description
Element Care is seeking a Project Manager, Clinical & Operational Innovations to lead initiatives that enhance care for frail seniors in our PACE program. In this role, you will plan and manage cross-functional projects that improve clinical quality, streamline operations, and support seniors living safely in their communities. You’ll collaborate with interdisciplinary teams, analyze workflows and data, design and implement new care models, and track outcomes. This position offers the opportunity to drive meaningful change in a compassionate, patient-centered, community-based environment.Lead clinical and operational innovation projects from planning through implementation for PACE programs
Coordinate cross-functional teams including clinical, operations, IT, and administration
Analyze workflows, care processes, and data to identify improvement opportunities
Develop project plans, timelines, and resource requirements; manage risks and issues
Standardize and document new processes, protocols, and performance metrics
Partner with clinical leaders to pilot and scale new care models and services
Monitor project outcomes, quality indicators, and ROI; prepare reports and presentations
Engage frontline staff and stakeholders to ensure adoption and change management
Ensure compliance with relevant healthcare, payer, and PACE regulations
Support continuous improvement culture aligned with Element Care’s mission and values
