Key Responsibilities
- Conduct both virtual and in-person comprehensive assessments of clients to identify their unique needs, strengths, and barriers. For instance, assess a new client virtually to gather historical information and schedule an in-person follow-up for deeper engagement.
- Design and implement individualized treatment plans using evidence-based therapeutic methods, including cognitive-behavioral therapy (CBT) and motivational interviewing, tailored to address issues such as anxiety, depression, trauma, and substance use disorders.
- Engage in regular collaboration with interdisciplinary teams, including medical professionals and rehabilitation specialists, ensuring a seamless care continuum for clients.
- Document client progress and treatment modifications in an electronic health record (EHR) system, maintaining compliance with legal, ethical, and organizational standards.
- Serve as an advocate for clients, assisting them in navigating community resources, which may include arranging for housing, employment support, or additional therapeutic services.

Qualifications
